Key Responsibilities
- Coordinate care for clients living with disability
- Make sure each client has a person-centred support plan, built with families and health professionals
- Lead, train and resource the support worker team for complex needs
- Keep the service in line with NDIS regulations and government standards, including client funds
- Keep careful documentation and assessments for all clients
- Build strong ties with clients, families and key stakeholders
Requirements & Eligibility
- Enrolled Nurse or Registered Nurse with AHPRA registration
- Proven disability care experience, ideally under the NDIS framework, and/or aged care experience
- Strong leadership and people management skills
- Excellent interpersonal and communication skills with people from diverse backgrounds
- Commitment to high clinical care and compliance standards
- Understanding of the NDIS and experience with clients who have complex needs
- A current NDIS Worker Screening Check and a Working with Children Check
- A First Aid Certificate less than 3 years old
- A valid driver's licence
