You would look after the clinical side of residents' lives: thorough assessments, care plans that you write and keep reviewing, safe medicine rounds and careful notes. Practice stays within your scope and follows AHPRA requirements and aged care standards, with the Care Manager and General Manager as close colleagues. Key Responsibilities
- Complete full assessments, gathering health and care information from residents and families
- Contribute to admissions and to later assessments
- Write, apply and review resident care plans on a regular cycle
- Record clinical information clearly and on time
- Provide direct clinical care to residents
- Give medicines safely and manage their ordering, storage and documentation
- Monitor residents closely and respond to any change in clinical condition
- Work with residents, families, carers, GPs and the care team for good outcomes
- Join quality, compliance and continuous improvement activities
- Support and mentor care staff on residents' clinical needs
Requirements & Eligibility
- Current registration with AHPRA
- Residential aged care experience and/or knowledge
- Strong clinical assessment, medication management and documentation skills
- Understanding of aged care accreditation and compliance requirements
- Knowledge of government standards for aged care delivery
- Excellent communication and interpersonal skills
- Able to work in a multidisciplinary team
- A current National Criminal History Check, or the ability to obtain one
- Genuine passion for supporting older people to live with dignity, choice and respect, and able to meet the legislative requirements of Aged Care Worker Screening
