Your clients are older people receiving aged care at home and people supported by the NDIS. You will help them stay healthy and keep their independence. Each care plan is built around the client's needs and goals, so good listening matters as much as clinical skill.

Key Responsibilities
- Provide nursing care to aged care clients at home and NDIS participants
- Do clinical assessments and prepare and review care plans
- Support clients with medication and monitor their health for changes
- Keep clinical documentation accurate and up to date
- Work with clients, families, carers and healthcare professionals, and promote choice, dignity and independence
Requirements & Eligibility
- Current AHPRA registration as a Registered Nurse
- Relevant experience in community nursing, aged care or disability services
- Strong clinical assessment skills
- Strong communication and organisational skills
- Confidence working independently in a community setting
- A compassionate approach and a commitment to person-centred, safe care
- Willingness to move to the Northern Territory
