It supports older Australians in their own homes with person-centred care through the Support at Home program, and it wants someone caring and experienced for its close-knit team.
Your main focus is the client. You lead care coordination, look after individual support plans and work independently, because the team has a flat structure. Key Responsibilities
- Stay on good, respectful terms with clients and the people close to them
- Work hand in hand with support workers and external providers
- Line up support services that suit each client's goals and health conditions
- Hold assessments, then put care plans in place and update them when needed
- Keep client safety, wellbeing and quality of life as the deciding factors
- Head a team of Support Workers and check their work for quality and continuity
- Watch over client budgets and keep spending on track
- Make sure everything meets the law, funding conditions and in-house rules
- Write clear, correct progress notes promptly and go over what support workers, suppliers and allied health professionals provide
- Welcome each new client with an intake assessment and a meet and greet
Requirements & Eligibility
- A proven history of work in aged care, home care, community services or a health setting
- Sound assessment skills plus sound care planning skills
- Sound case management skills
- Confident handling your own workload and several competing priorities
- Excellent at communicating, building relationships and leading others
- Working knowledge of My Aged Care, and also of the Aged Care Quality Standards
- A driver's licence and a vehicle you own
- Willing to travel to clients across Sydney
- Bilingual or multilingual ability gives you a strong advantage, and a clinical background (nursing or allied health, say) is nice to have though not required
